Job Opportunity: Physical Therapist Assistant (PTA) - Home Health
Location
- Pomona, California
- Territory covers up to a 60-mile radius including Corona, Riverside, Moreno Valley, Norco, Eastvale, Jurupa Valley, Perris, Redlands
Employment Type
- Travel / Contract
- Contract Length: 13 weeks
Work Schedule
- Day shift, Monday to Friday (5 days/week)
- Hours: 8:00 AM to 5:00 PM (8 hours/day)
- Guaranteed 40 hours per week
Patient Load and Case Type
- Average 5-7 patients per day
- Minimum 6 visits daily or 30 points weekly
- Geriatric patient population
Required Qualifications
- Valid Physical Therapy Assistant license in California
- Minimum 1 year of recent Physical Therapy Assistant experience
- BLS (Basic Life Support) certification required
- Experience with OASIS
On-Call
- No on-call required
Equipment Provided
- iPad provided for work
- Cell phone use not allowed for work purposes
Mileage
- Mileage reimbursed at IRS standard rate
Compliance and Documentation Requirements
- Background checks within 90 days of start, including SSN trace, county searches (7 years), FACIS Level 1, nationwide sex offender, motor vehicle records, education verification, and work history verification
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- State-specific background checks as applicable
- Fingerprinting requirements vary by state (not required for all placements)
- Education verification of highest degree related to profession
- Signed Orientation Safety Manual Acknowledgement (updated annually)
- Primary source verification of state license within 90 days of start and upon expiration
- Competency exams and medication/pharmacology tests as applicable
- Agency background attestation required prior to start
- Facility-specific Acceptable Use Policy must be completed before start
- Flu and COVID vaccination or facility-specific declination documentation required
- Physical exam within last 12 months (not required annually)
- Fit mask test (N95 accepted) required
- Negative TB test or chest X-ray within 1 year; TB questionnaire annually (required for positive TB history)
- Immunizations or titers required for Measles, Mumps, Rubella, Hepatitis B, Varicella (with declination or booster options)
- 9-panel drug screen within last 12 months (instant acceptable)
- Two professional references within 2 years of start (travel, allied, per diem acute care)
- Full 7 years of work history documented with no gaps on employment application
- Work history verification for 7 years required within 90 days of start (attestations accepted after 3 failed attempts)
- Annual skills checklist required
