Physical Therapist - Home Health (Travel/Contract)

Location: Walla Walla, Washington
Contract Length: 13 weeks


Job Details

  • Schedule: Monday to Friday, Day shift (8:00 AM - 5:30 PM)
  • Hours: 40 hours per week, guaranteed 30 patient visits weekly
  • Average Patients per Day: 5-7
  • Patient Population: Geriatric cases
  • Territory: Up to 60-mile radius covering parts of Walla Walla, Columbia, and Umatilla counties
  • Documentation: All charting must be completed by 9:00 AM the following morning
  • Dress Code: Scrubs (any color), no jeans
  • Equipment Provided: iPad
  • Cell Phone Use: Not allowed during work
  • Mileage Reimbursement: At IRS standard rate

Required Qualifications

  • Valid Physical Therapist license in Washington
  • Minimum 1 year of Physical Therapist experience within the last year
  • BLS (Basic Life Support) certification required
  • Experience with Homecare Homebase (HCHB) EMR required

Compliance & Background Requirements

  • Background checks within 90 days of start, including:
    • SSN Trace
    • County searches (last 7 years)
    • FACIS Level 1
    • Nationwide Sex Offender
    • Motor Vehicle Records (MVR)
    • Education and work history verification
  • EPLS/GSA/SAM and HHS/OIG checks required within 90 days
  • Fingerprinting required if candidate has not lived in Washington for the past 3 years (completed on-site)
  • State-specific criminal history and employability status checks as applicable
  • Education verification of highest degree related to profession
  • Nursys verification as applicable

Health & Safety Requirements

  • Physical exam within the last 12 months (not required annually)
  • Fit test for N95 mask (no preferred model)
  • Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
  • Annual TB questionnaire required
  • Immunizations or proof of immunity for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • 9-panel drug screen within 12 months (instant acceptable)
  • Seasonal flu vaccination or facility-specific declination form required
  • COVID vaccination or facility-specific declination required

Additional Requirements

  • Proof of current auto insurance required due to driving responsibilities
  • Signed orientation safety manual acknowledgment (updated annually)
  • Completion of facility-specific acceptable use policy prior to start
  • Two professional references within 2 years of start (travel/allied/per diem acute care)
  • Full 7 years of work history documented with no gaps in employment application
  • Annual skills checklist and competency exams as applicable

  • This position does not allow first-time travel therapists; prior travel experience is mandatory.
  • Mileage reimbursement and travel pay rate apply to local candidates.
  • Strict adherence to documentation deadlines and communication protocols is required.
