Registered Nurse (RN) - Long Term Care / Skilled Nursing Facility

Location: Winnemucca, Nevada
Contract Length: 13 weeks

Job Overview

• Facility Type: Nursing home, skilled care with 34 beds (currently 22 residents), all semi-private rooms
• EMR System: Cerner
• Patient Ratios:

  • Days: 1 RN per 11 patients
  • Nights: 1 RN per 22-30 patients
  • CNA Ratios: Days 1:6, Nights 1:6 or 1:8
• Common Diagnoses: COPD, diabetes, cognitive deficits, multiple comorbidities
• Shifts: 12-hour shifts (7:00 AM - 7:30 PM)
• Weekend: Typically every other weekend
• Holidays: May be required to participate in holiday rotation
• Scheduling: Modified self-scheduling; flexibility required to cover staffing gaps
• Orientation: One shift of orientation with a preceptor
• Floating: May be required to float to memory care unit if needed

Job Duties

• Medication administration and simple IVs/tubes (no complex IVs or titratable drips)
• Prefer candidates with leadership experience such as charge nurse duties
• Wound care managed by PT; nurses assist as needed
• Interaction with patients with cognitive deficits, including redirection techniques without confrontation
• No ventilators or tracheostomies on site
• Nurses administer respiratory therapy and manage home CPAP/BiPAP equipment

Requirements/Qualifications

• Valid RN license in Nevada
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Minimum of 1 year of skilled nursing experience, including recent experience within the last year
• Experience in nursing home settings
• Current BLS certification

Additional Information

• Physician availability after hours with NP coverage Monday-Friday and MD oversight
• Interdisciplinary team includes PT/OT/SLP, DON, Resident Care Coordinator, MDS Coordinator, Resident Advocate, Dietician, Finance, NP, and Medical Director as needed
• Pharmacy on-site with on-call coverage at night; medication dispensing via Omnicell system
• No call or standby requirements
• Scrub color: Navy blue for RNs
