Hospice Nurse

Overview

• Location: Sunnyvale, California
• Employment type: Contract
• Contract length: 13 weeks
• Schedule: Days, 9:30am–6:00pm; schedule can be flexible based on patient availability
• Weekend requirement: One weekend per month
• Service area: Millbrae, Burlingame, San Mateo, Foster City, Belmont, Redwood City, and nearby cities as needed

Role Summary

• Admit patients to hospice service
• Assess physical, psychosocial, environmental, safety, and developmental needs
• Develop individualized plans of care
• Document observations and interventions related to the patient’s environment
• Coordinate care with a multidisciplinary team
• Participate in quality assessment and performance improvement activities
• Conduct follow-up visits as needed

Required Qualifications

• Valid RN license in California
• Associate of Science in Nursing / Associate Degree in Nursing
• Hospice experience within the last year
• 1 year of hospice experience
• BLS certification
