Job Description
This position is within a 40-bed rehabilitation hospital specializing in physical medicine and rehabilitation. The hospital provides customized, intensive rehabilitation for patients recovering from stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury. The facility is CARF-accredited for Stroke Specialty, Comprehensive Integrated Inpatient Rehabilitation, and Amputation Specialty Programs.
As a Case Manager RN, you will be part of a team focused on supporting patients throughout their recovery journey with specialized treatment plans tailored to individual needs.
Required Qualifications
- Valid Registered Nurse (RN) license in Wisconsin
- Bachelor of Science in Nursing (BSN)
- Minimum 2 years of experience as an RN in Case Management
- At least 2 years of Case Management experience, including experience within the last year
- Experience working in a Rehabilitation setting
- Must hold at least one of the following certifications: LCSW, PT, SLP, or OT
