Job Description

• Coordinate, facilitate, and execute social work functions with patients through collaboration with the Interdisciplinary Care Transitions (ICT) team.
• Follow patients throughout the care continuum, identifying and addressing psychosocial needs.
• Provide ongoing support via assessment, individualized treatment plans, continuous evaluation, case management, mediation, referral, consultation, education, and advocacy.
• Enhance patient management and satisfaction by integrating case management, discharge planning, and social work practices.
• Act as a patient advocate, investigate and report adverse occurrences, and provide staff education related to discharge planning and psychosocial healthcare aspects.
• Advocate for understanding physical, biological, psychological, emotional, and environmental factors affecting patient health.

Requirements

• Certification: BLS certification (American Heart Association) required.
• Education: Master’s Degree in Social Work, Marriage and Family Therapy, or Professional Counseling.
• Licensure: California State license in Social Work required.
• Experience: Minimum 3 years recent experience as a Social Worker in a hospital setting.
• Experience with hospice, palliative care, code status, advance directives, FMLA/disability paperwork, IHSS, and emergency visas preferred.

Schedule

• Monday to Friday
• 8 hours per day
• Flexible start time between 8:00 AM and 9:00 AM
