Employment Type: Long Term
Contract Length: 13 weeks
Shift: Day shift (Monday to Friday, AM shift)

Provide home hospice care within a coverage area approximately a 60-mile radius, primarily San Bernardino territory (excluding Victorville).
May be asked to assist in other territories within the service area as needed.
Productivity expectation: 24-30 patient visits per week, based on a 40-hour workweek.
Overtime requires prior written approval from the Administrator.
Two weeks cancellation notice required.
