Temporary contract
Contract length: 13 weeks
Schedule: Monday to Friday, 8:00 AM – 4:30 PM (8 hours/day, 40 hours/week)
On-call/weekend requirement: 1 weekend every 6 weeks, 1 weekday on-call every week

Provide and direct nursing care to patients in their homes as prescribed by physicians
Coordinate total plan of care with healthcare professionals, patients, families, and community resources
Ensure continuity of patient care through planning and communication
Perform duties within a Point of Care setting
