Nursing Contract Opportunity - St Albans, Vermont

Job Overview

  • Hospice coverage required when on-call.
  • Serve as Case Manager for assigned patients, managing caseloads and visit times according to agency productivity standards.
  • Utilize the nursing process to assess patient and caregiver needs and provide quality care.
  • Develop and communicate realistic plans of care in collaboration with patients and caregivers.
  • Implement medical plans of care, nursing interventions, and procedures to ensure safe, effective, and quality care.
  • Maintain up-to-date knowledge and skills in high-tech nursing areas such as IV use and maintenance, wound VAC, and complex wound care.
  • Collaborate with LPNs within their scope of practice, delegate tasks, and ensure patient needs are met.
  • Evaluate and document patient responses and care outcomes to ensure quality care delivery.
  • Maintain accurate and timely documentation in compliance with state, federal, and agency regulations.
  • Follow agency protocols for documentation systems and visit management.
  • Provide patient and caregiver education on medication side effects, treatment options, discharge instructions, and reportable conditions.
  • Identify and communicate interdisciplinary patient needs (e.g., physical therapy, social work).
  • Complete required agency, state, and federal forms accurately and timely.
  • Conduct visits efficiently and communicate scheduling needs promptly.
  • Assess patient needs for Home Care Aide (HCA) services and coordinate ongoing supervision.
  • Report patient incidents (infections, injuries) as necessary.
  • Coordinate safe patient discharges and transfers, ensuring proper documentation.
  • Collaborate with physicians, providers, team members, and vendors to support the plan of care.
  • Participate in monthly peer review audits and provide/accept constructive feedback professionally.
  • Schedule and facilitate case conferences to enhance team communication.
  • Advocate proactively for patient care to ensure sensitivity to individual patient and family needs.

Requirements

  • Must be licensed and practice in accordance with Vermont State Board of Nursing regulations.
  • Knowledge of and compliance with State and Federal regulations and agency policies.
  • Experience with OASIS documentation and McKesson processes.
  • Ability to work collaboratively within a multidisciplinary team.
  • Prior travel experience is not specified or required.
