Case Manager Travel Position

Location: Springfield, Oregon
Contract Length: 13 weeks


Job Summary

Seeking a Case Manager with experience in adult physical rehabilitation to support care management in an inpatient rehabilitation setting. The role focuses on discharge planning, coordination, and managing a caseload of adult neuro diagnoses.


Work Details

  • Employment Type: Travel
  • Shift: Day
  • Hours per Week: 32 (40 guaranteed weekly hours noted in unit profile)
  • Weekend Hours: None mandatory
  • Call Requirements: None
  • Patient Population: Adults with neurological diagnoses such as CVA, SCI, TBI, neurodegenerative diseases, polytrauma, amputation, general medical, deconditioning, cardiac, and simple orthopedic conditions
  • Caseload: Approximately half of the census (27 beds total)
  • EHR System: Epic
  • Dress Code: Casual business attire (no blue jeans)

Required Qualifications

  • Valid RN license in Oregon
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Minimum of 1 year of rehabilitation experience, including experience within the last year
  • Experience working with adult patients
  • Experience in a rehabilitation setting
  • Care Management experience or certification
  • Experience as a Care Manager

Additional Information

  • No float or call requirements
  • CMA staff available to assist with delegated administrative tasks as needed
  • Nursing patient ratio approximately 1:5
  • No prior travel experience explicitly required
