Job Summary
The Registered Nurse (RN) will provide direct patient care services to hospice patients and supportive care to their families. Responsibilities include managing an interdisciplinary team, case managing patient clinical records, and ensuring the highest quality of hospice care in accordance with the Plan of Care. The RN will perform initial and ongoing patient assessments, develop and implement hospice care plans, and collaborate closely with the interdisciplinary team to address physical, psychosocial, educational, emotional, and spiritual needs, including bereavement support.
Additional duties include:
- Reporting changes in patient condition to physicians
- Maintaining accurate and updated medical records
- Practicing infection control and standard precautions
- Monitoring staff scheduling and participating in quality improvement programs
- Participating in on-call schedules as needed
- Supervising team members and conducting routine visits
- Educating patients, families, and caregivers on care needs and hospice philosophy
- Acting as the primary liaison between physicians, patients/families, and the hospice team
Contract Length
Approximately 14 weeks
Required Qualifications
Experience
- Minimum of 1 year hospice experience within the last year
- Experience in acute care, oncology, long-term care, and home health settings
- Knowledge of managed care principles
- Knowledge of regulatory guidelines including Medicare, Medicaid, and ACHC
Education
- Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
Certifications & Licensure
- Valid Registered Nurse (RN) license in South Carolina
- BLS certification issued by the American Heart Association
- Valid driver’s license and current automobile insurance

- Participation in compliance training and staff meetings is required
- Prior travel experience is not specified or required for this position
