Job Summary

The Registered Dietitian is responsible for developing and implementing a nutrition services program in collaboration with an Interdisciplinary Team (IDT). Key duties include conducting nutrition assessments, delivering nutrition counseling, overseeing menu planning and meal delivery, coordinating supplements, and ensuring compliance with safety and regulatory requirements. The role involves working closely with healthcare providers to support care transitions and participating in team meetings and quality improvement activities.

Duties and Responsibilities

Conduct nutrition assessments to determine participant needs, preferences, and goals.
Participate in IDT meetings to develop participant care plans.
Deliver and document nutrition interventions, including special diets and counseling.
Oversee menu planning and meal delivery to ensure safety and regulatory compliance.
Coordinate supplements as ordered.
Provide staff training on meal delivery programs as needed.
Support care transitions between settings (e.g., hospitals, skilled nursing facilities, home).
Assist with intake and enrollment processes.
Monitor and report changes in participant condition or psychosocial status.
Maintain confidentiality and comply with all policies and procedures.
Work varied schedules including evenings, nights, and weekends as needed.
Complete required documentation, trainings, and occupational health screenings timely.
Perform other duties as assigned.

Required Qualifications

Valid Registered Dietitian Nutritionist (RDN) license in California.
Registered Dietitian Nutritionist (RDN) with a minimum of 4 years of experience managing nutrition services in a similar setting.
Experience working as an RDN within the last year.
Basic Life Support (BLS) certification.
Experience working with geriatric patients, including at least 1 year with a frail or elderly population.
Experience mentoring and developing team members.
