Job Opportunity: Clinical RN - Home Health RN

Job Summary

• Conduct home health nursing assessments to determine nursing, personal care, and equipment needs in the home.
• Participate in interdisciplinary teams as necessary.
• Regularly re-evaluate participant nursing needs and update care plans accordingly.
• Develop and initiate care plans based on nursing diagnoses, incorporating therapeutic, preventative, and rehabilitative nursing actions.
• Deliver and document nursing interventions, including medication administration and ordered tests/treatments.
• Instruct participants, families, and caregivers on disease processes, self-care, and prevention strategies.
• Communicate changes in participant condition promptly to the interdisciplinary team.
• Manage care transitions between settings (hospitals, skilled nursing facilities, home, etc.).
• Identify and implement discharge planning needs.
• Supervise and evaluate home health aide care every two weeks.
• Participate in end-of-life care coordination and support.
• Provide backup coverage for other nursing functions as needed.
• Work varied schedules including evenings, nights, weekends, and overtime.
• Complete all required documentation, trainings, and occupational health screenings timely.
• Maintain confidentiality and follow company policies and quality improvement activities.

Required Skills & Experience

• Nursing knowledge and skills to treat frail, elderly participants and manage complex clinical situations.
• Ability to initiate venous access for IV hydration or lab draws; experience with IM, SC, or transdermal injections and wound care.
• Highly motivated, self-directed, able to make sound decisions in emergency situations.
• Excellent clinical, organizational, and communication skills with seniors, families, and interdisciplinary teams.
• Ability to work independently with minimal supervision and prioritize in a fast-paced environment.
• Experience working with diverse populations.
• Commitment to supporting vulnerable seniors.

Preferred Skills & Experience

• Minimum of 2 years nursing experience in a clinical setting with frail or elderly populations.
• Home health agency experience preferred.
• Bilingual English/Spanish preferred.

Education & Licensure

• Graduate of an accredited school of nursing required.
• BSN preferred.
• Unencumbered Registered Nurse (RN) license required.
• Must meet qualifications established by the home health agency and licensure regulations.

Additional Information

• Must be legally authorized to perform nursing duties in California.
• Must act within the scope of authority and comply with California Nursing Practice Act.
