Position Details

Required Credentials & Certifications

  • Licenses:

    • Valid professional nursing license verified through Nursys
    • Primary source verification of license/certification within 30 days of start
    • Driver’s License, State ID, or Passport
  • Certifications:

    • BLS (Basic Life Support) – American Heart Association or Red Cross
    • ACLS (Advanced Cardiovascular Life Support) – American Heart Association or Red Cross
  • Exams & Checklists:

    • Prime Core Competencies Exam (minimum 80%)
    • Prime Skills Checklist
    • Prime Specialty Exam (minimum 80%)
  • Other Professional Requirements:

    • Prime Education Verification
    • Prime OIG (Office of Inspector General) check
    • Prime Criminal Background Check (national, within one year of start)
    • Prime SAM/EPLS exclusion search
    • Prime Agency Attestation

Health & Immunization Requirements

  • COVID-19 Vaccine: 2 doses plus booster or completed declination
  • MMR: Proof of 2 vaccines at least 28 days apart or positive titer
  • Hepatitis B: Required (declinations not accepted)
  • Influenza Vaccine: Required or declination (for flu season Nov 1 - Mar 31)
  • TB Test: Within 1 year of start date
  • Varicella: Proof of 2 vaccines at least 28 days apart or positive titer
  • Tdap: Required (declinations not accepted)
  • Physical/Health Statement: Must be completed within 60 days of start and state clearance to work
  • Positive PPD with Negative Chest X-Ray: If applicable, negative chest x-ray within 3 months of positive test required
  • Annual Respirator Fit Test: Optional, but if required must be completed prior to start

Drug Screening

  • Minimum 10-panel drug screen including THC within 60 days of start
  • For California locations only: 10-panel urine drug screen excluding THC plus psychoactive THC saliva test
