Job Opportunity: Registered Nurse - Hospice
Location: Leominster, Massachusetts
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits with an average patient load of 5-8 patients per day.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Must have reliable transportation, a current driver’s license, and agency-required liability insurance.
- Travel required up to 75 miles from home clinic; approximately 150-200 miles traveled weekly.
- Work schedule is Monday through Friday, 8 AM to 5 PM.
- No guaranteed hours; floating to nearby clinics may be required if patient census is low.
- Weekend and on-call requirements vary; discuss during interview.
- Charting proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs).
- Dress code: Scrubs.
Required Qualifications
- License: Current Registered Nurse (RN) license for Massachusetts (license must be in hand; no pending licenses accepted).
- Experience: Minimum 1 full year of recent Adult Home Health or Hospice experience in the specialty.
- First-time travelers not accepted; must have recent specialty experience.
- CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
- Proficiency with HomeCare HomeBase (HCHB) within the last 6 months; retraining required if last used over 6 months ago.
- RNs must be proficient in OASIS documentation.
- Must have reliable transportation and valid driver’s license.
- Must be willing to travel within a 75-mile radius of the home clinic.
Productivity Expectations
- Weekly productivity units: 25-30
- Patient visits per day: 5-8
Additional Requirements & Compliance
- Background checks including seven-year history, sex offender search, and state-specific screenings as required.
- Drug screening (10-panel) required.
- Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months, updated annually.
- Immunization or titer proof for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella.
- COVID-19 vaccination or declination documentation required.
- Must complete various compliance documents and training prior to start, including but not limited to:
- Confidentiality and privacy policies
- HIPAA training
- Hospice Core Care Policy Quiz
- Code of Conduct
- COVID-19 Policy and Attestation
- Personal or auto insurance policy required for hospice and home health placements.
Travel & Work Environment
- Home health/hospice setting with home visits.
- Must have reliable transportation for travel between patient homes.
- Mileage reimbursed at IRS standard rate (patient-to-patient travel only).
- Some positions may have alternative shift schedules (e.g., 4x10s Friday-Monday), but most are standard 5x8 Monday-Friday.

- Must be proficient in required EMR systems: HomeCare HomeBase (HCHB) and Point Care.
This position is ideal for experienced hospice RNs who are comfortable with home visits, documentation, and managing a patient caseload independently within a travel contract setting.
