Physical Therapy Assistant - Home Health (Travel/Contract)
Location: Tallahassee, Florida
Contract Length: 13 weeks
Shift: Day shift, Monday to Friday, 8:00 AM – 5:00 PM (5x8 hours)
Hours per Week: 40
Job Overview
- Provide physical therapy assistant services in home health settings.
- Patient visits include both home visits and care facilities.
- Average caseload: 5-10 patients per day (approximately 6-8 points per day based on visit/activity type).
- Types of cases: Start of Care and Follow Ups.
- Travel required up to 75 miles from home clinic; expected travel 250-300 miles per week.
- Use of EMR systems: HomeCare HomeBase, Outlook, Microsoft Teams.
- No guaranteed hours; if patient census is low, may be floated to nearby clinics to pick up hours.
- Weekend and on-call requirements vary; discuss during interview.
Required Qualifications
- Valid Physical Therapy Assistant license in Florida.
- Minimum of 1 year of recent Physical Therapy Assistant experience within the last year.
- Experience working with adult patients in a home health setting.
- Current BLS certification.
Compliance and Documentation Requirements
- Background checks covering the last 7 years of residence/work/school, including SS trace, county, nationwide, sex offender, and state-specific searches.
- Drug screening: 10-panel drug test required.
- Physical exam within 12 months confirming good health and no communicable diseases; updated annually.
- Negative TB test or chest X-ray within 12 months; updated annually.
- Immunization or titer proof for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella.
- Seasonal flu vaccination or documented declination required.
- COVID-19 vaccination series or declination required.
- Education verification for applicable degree.
- Work history verification for at least two most recent employers or last 7 years.
- Two professional references required.

- Mileage reimbursed at IRS standard rate; compensation is from patient home to patient home (commute to/from first and last patient is not billable).
