Behavioral Health Registered Nurse Travel/Contract Job - Hazard, Kentucky

Position Overview

• Specialty: Registered Nurse – Behavioral Health
• Location: Hazard, Kentucky
• Employment Type: Travel/Contract
• Contract Length: 13 weeks
• Shift Options:

  • 3 shifts per 2-week pay period (36 hours total) of 12 hours each (7:00 AM – 7:30 PM)
  • 4 shifts per 2-week pay period (48 hours total) of 12 hours each (7:00 AM – 7:30 PM)
• Schedule: Every other weekend required; blocked scheduling cannot be accommodated
• Float required within scope of practice
• RN candidates must reside within 75 miles of the facility to be considered
• Orientation occurs bi-weekly
• Start date is immediate upon job offer acceptance

Required Qualifications

• Valid RN license in Kentucky
• Minimum 2 years of Behavioral Health nursing experience, with experience within the last year
• Current Basic Life Support (BLS) certification issued by the American Heart Association
• Crisis Prevention Institute (CPI) certification
• NIH Stroke Scale (NIHSS) certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Experience with MEDITECH Electronic Medical Record (EMR) system

Required Health and Compliance Documents

• Annual physical exam or physician note within 1 year prior to start, stating candidate is cleared to work without restrictions
• Negative TB test via QuantiFERON blood test within 1 year prior to start (T-Spot/PPD NOT accepted)
• Proof of immunizations or titers for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Varicella
  • Hepatitis B (series, titer, or declination within 1 year)
  • Tetanus/Diphtheria/Tdap (Td not accepted; declinations acceptable if within 1 year)
• Annual drug test through specified 9-panel bundle (no THC) with expanded opiates and oxycodone, completed within 30 days of start
• Fit mask test to be completed onsite; PAPRs not accommodated due to facial hair; medical or religious exemptions allowed but must be submitted on day one
• Seasonal flu vaccination or approved medical/religious exemption required during flu season (9/1–3/31); exemption requests must be submitted within 48 hours of offer acceptance and approved prior to start

Background and Verification Requirements

• Background check per Appalachian Regional Bundle with 9-panel drug screen and expanded opiates, including:

  • National criminal search
  • Social Security and address trace
  • Sex offender and fugitive databases
  • County searches for all counties in last 7 years
  • FACIS 3 or equivalent
  • EPLS/GSA/SAM and HHS/OIG checks within 30 days prior to start
• Education verification included in background or third-party verification
• NURSYS verification prior to start
• Kentucky Vulnerable Adult Maltreatment Registry verification within 30 days prior to start (annual update required)

Additional Requirements

• Signed and dated job description acknowledgment, renewed annually
• Completion of facility-specific employee health attestation and instructions prior to compliance deadline, including uploading required health documentation screenshots (TB results, TB risk assessment, OSHA respirator medical evaluation, flu vaccine/exemption)
• Annual skills checklist completion
• Must submit unexpired government-issued photo ID (driver’s license or state ID)
• Must sign assignment confirmation within 24 hours of offer acceptance

• Candidates must not be submitted to more than one job at a time
• Candidates must submit with required coversheet and copy of current valid driver’s license
• PAPRs cannot be accommodated due to facial hair policy; candidates must shave if exemption denied
• Blocked scheduling is not possible; flexibility required for floating within scope of practice
