Job Description

• Provide home health physical therapy services across multiple areas including Richmond, Rodeo, El Cerrito, Hercules, Oakland, Berkeley, Alameda, and surrounding cities.
• Shift: 8-hour day shifts (8:00 AM - 4:30 PM), 5 days per week.
• Expected to see 5-6 patients per day with a 1:1 patient ratio.
• Guaranteed 36 hours weekly, though guaranteed hours do not apply during the first 2 weeks (ramp-up, holidays, time off).
• Weekend patient visits are optional to meet productivity goals.
• No on-call responsibilities.
• Own transportation is required; occasional travel radius may exceed 50 miles.
• Documentation and timely communication are required; documentation tools will be provided.

Required Qualifications

• Valid Physical Therapist license in California.
• Minimum of 1 year of Physical Therapist experience, including experience within the last year.
• Experience in a Home Health setting.
• Completion of at least one travel assignment as a Physical Therapist.
• Current Basic Life Support (BLS) certification (American Heart Association or American Red Cross).

Compliance and Onboarding Requirements

• Government-issued photo ID (Driver’s License or State ID).
• Emergency contact form.
• I9 attestation form.
• Background checks including 7-year search, nationwide, sex offender registry, and annual EPLS/GSA/SAM and HHS/OIG checks.
• Certifications: BLS required; ACLS and PALS preferred but not mandatory.
• Physical exam within 1 year prior to start, confirming clearance to work without restrictions.
• Immunizations and health screenings:

  • Tetanus/Diphtheria/Tdap vaccine within 10 years or annual declination.
  • Negative TB test or chest X-ray within 1 year.
  • Measles, Mumps, Rubella (MMR) immunity via vaccines or positive titers within required timeframes.
  • Hepatitis B immunity or declination updated annually.
  • Varicella immunity via vaccines or positive titer.
  • Seasonal flu vaccine or declination required year-round.
  • COVID-19 vaccination documentation or facility-specific declination form required.
• Drug screening: 10-panel drug screen within 1 year of start.
• Auto insurance and registration must be current and maintained throughout the assignment.

• First-time travel therapists are not accepted for this position.
• Locals are accepted.
• Facility uses MediScrib & OASIS documentation systems; training provided.
• Dress code: Any color scrubs.

• Mileage reimbursement at IRS standard rates.
