Position Details

  • Specialty: Therapy/Rehabilitation (Outpatient OT)

  • Contract Length: 13 weeks

  • Weekend Requirement: One weekend day (typically Sunday) every other weekend

Required Certifications and Licenses

  • BLS (Basic Life Support)
  • California OT License
  • Other certifications may be required based on unit or specialty (e.g., ACLS, PALS) – verify with clinical track if unsure.

Compliance and Documentation Requirements

  • Background Check:

    • Must be within 45 days of start date, covering all locations lived/worked in the past 7 years.
    • Includes SSN Trace, Death Master Index, Counties, Nationwide, National Sex Offender, FACIS Level 3, Education Verification, MVR (if applicable), and international background if applicable.
    • Returning candidates with less than 30-day gap may not require new background check if current and acceptable.
    • Open/active felony cases will disqualify.
  • Education Verification:

    • Required for all candidates.
    • RNs/LVNs must verify nursing degree; Allied must verify related degree or highest education level.
    • International education requires additional documentation (CES report).
  • References:

    • Minimum 2 supervisor references covering 1 year of employment within the last 3 years in a comparable specialty/setting.
    • For CNAs, 6 months of employment references required.
  • Employment Application:

    • Must include at least last 5 years of employment with month/year format.
  • Physical Exam:

    • Within 1 year of start, signed by MD, NP, PA, or DO. Must indicate ability to work with or without accommodation.
  • Drug Testing:

    • 10-panel drug screen within 90 days of start (no instant tests). Negative results only.
  • Immunizations and Health Requirements:

    • TDAP (within 10 years or declination accepted)
    • Negative TB test or chest X-ray within 12 months (annual updates required)
    • MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer or declination
    • Hepatitis B vaccination or signed declination
    • Varicella - 2 vaccines or positive titer or declination
    • Flu vaccine during flu season or declination
    • COVID vaccine or declination
    • Respirator clearance (RespSafety) for travel candidates starting 2/5/2024 or later
  • Fit Mask Testing:

    • Required for travel candidates starting 2/5/2024 or later.
    • Must have valid fit test within 1 year or complete onsite.

Work and Scheduling

  • Floating between units (Acute Rehab and inpatient floors) is required.
  • One weekend day every other weekend (typically Sunday).
  • Block scheduling may be accommodated at manager discretion.
  • Locals are accepted; first-time travelers are not.
