Job Description

• Provide one-on-one physical therapy treatment to 8-9 patients per day with point-of-service documentation.
• Flexibility to float between outpatient and inpatient settings within the hospital system, including Alameda Hospital Outpatient, Alameda Hospital Inpatient, Fairmont, and SLH inpatient as needed.
• No weekend shifts at Alameda Outpatient; occasional Saturday shifts possible if floating to inpatient.
• Experience required: 1-2 years preferred (1 year considered but 2 years preferred).
• Specialty focus: Orthopedic and Neurological therapy; candidates with stronger orthopedic experience will be considered.
• Required certifications: Basic Life Support (BLS) and valid State license.
• First-time travelers accepted, but not new graduates combined with first-time traveler status.
• Return-to-Office (RTO) details to be discussed before contract signing; no changes accepted after contract is signed.

Required Credentials & Compliance

Licensure & Certifications

• Valid State License (unencumbered, current)
• Basic Life Support (BLS) certification (AHA only)
• Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Program (NRP), and other certifications may be required depending on unit assignment.
• Orientation Safety Manual Acknowledgement required.

Background Checks

• Comprehensive background check within 90 days prior to start, including:

  • SSN Trace
  • 7-year Nationwide, County, and National Criminal searches
  • FACIS Level 3
  • National Sex Offender Registry
  • OIG Sanction Search
  • EPLS/GSA/SAM verification
• No pending background checks accepted at start.

Health & Immunization Requirements

• Physical exam within 90 days of start, signed by MD, DO, PA, NP, or APRN confirming good health and ability to perform job duties.
• Fit mask test within 90 days of start (specific approved masks required).
• Tetanus/Diphtheria/Tdap vaccination within 10 years or signed declination (TD not accepted).
• Negative TB test or chest X-ray updated annually with specific testing protocols.
• Annual TB questionnaire regardless of TB test results.
• Proof of immunity or vaccination for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B (positive titer or declination)
  • Varicella
• Seasonal flu vaccination or signed declination required annually by November 6. Masking required during flu season regardless of vaccination status.
• Drug screening (oral saliva test) within 30 days of start; rapid tests not accepted.

Competency & Facility Requirements

• Completion of facility-specific competencies and new employee orientation modules on the first day of contract.
• Competencies must be completed within 14 days of start or assignment will be terminated.
• Color blindness testing within 90 days of start (Ishihara test accepted).
• Medication/pharmacology competency exams as applicable.
• Photo for badge (passport style, no filters) required.

Employment History & Documentation

• Current resume including last 7 years of employment.
• Verification of past 3 years of employment required.
• References required per agency standards (not facility requirement).

Additional Notes

• Floating assignments may require occasional weekend shifts.
• Candidates must be able to meet all credentialing, health, and compliance requirements prior to start.
• No new requests for changes after contract signing.
