Overview
- Travel/Contract assignment in Lacey, Washington
- 13-week contract
- Day shift: 5x8 hours, 8:00–16:30
- Home Health setting
- Geriatric patient population
- Territory up to a 60-mile radius from the hub/office
- Locals accepted at travel rate
Schedule and Work Expectations
- Monday–Friday with flexibility to work weekends
- Expected to be flexible with territory
- Timely documentation and communication required
- Average census: 5–7 patients per day
- Productivity requirement: 30 points weekly
Required Qualifications
- Valid LPN / LVN license in Washington
- Home Health (1 year of experience)
- Home Health experience within the last year
- One completed travel assignment
- BLS issued by American Heart Association or American Red Cross
- Driver's License
- Experience with Homecare Homebase
- Experience with OASIS
Preferred / Additional Notes
- iPad provided
- Mileage reimbursed at the IRS standard rate
- Scrubs of any color; jeans not allowed
- Cell phone not provided
