Job Opportunity: RN Telemetry I

Location: Monroe, Louisiana
Employment Type: Long-Term Assignment (LTA)

Job Description

• Must have prior telemetry experience.

Required Credentials & Compliance

Certifications

• BLS (AHA or ARC only) – Required for onboarding
• ACLS (AHA only) – Required for onboarding

Licenses

• Active professional nursing license verified through primary source within 30 days of start (Nursys verification required)
• Licensure must be active for the State of Louisiana

Professional Documents

• Resume/Profile – Required for submission
• HRIS Form – Required for submission
• Contract Annual Education Packet – Required for onboarding
• Badge Photo (forward-facing, well-lit, neutral background, JPEG) – Required for onboarding

Background & Screening

• Criminal Background Check – Must be completed within 30 days prior to start, including:

  • SSN Address Trace report (7 years)
  • County criminal searches for all counties lived/worked in last 7 years (state checks not accepted)
  • National criminal check including NSO, OFAC, OIG, and SAM
  • Education verification (3rd party)
  • Employment verification (last 5 years)
  • Driving record (included or separate)
• Drug Screen – Negative 10-panel within 30 days prior to start (includes marijuana/THC, PCP, amphetamines, opiates, cocaine, benzodiazepines, barbiturates, methadone, propoxyphene, methaqualone)

Health & Immunizations

• COVID-19 Vaccine – Optional; proof or exemption accepted
• MMR – Proof of 2 immunizations, positive titer, or declination with signed statement
• Hepatitis B – Proof of 3 immunizations, positive titer, or declination with required signatures
• Influenza Vaccine – Required during flu season (10/1 to 4/30); medical/religious exemptions accepted with proper documentation
• TB Test – Negative QuantiFERON blood test within 1 year or 2-step skin test; if positive, MD note or proof of treatment required
• Health Statement – Signed and dated within 1 year confirming ability to work without restrictions
• Varicella – Proof of 2 immunizations, positive titer, or declination with signed statement
• Tdap – Proof of vaccination within past 10 years or declination with required signatures
• Annual TB Questionnaire – Required within 6 months of start; if positive, documentation of evaluation and treatment completion required
• Eye Exam & Color Vision – Ishihara Color Test and Snellen Vision test within 1 year prior to start

Competency & Skills

• Skills Checklist – Unit-specific, within 1 year prior to start, updated annually
• Competency Exam – Passing score of 80% or higher within 1 year prior to start
• Core Competencies – Must be current within 1 year prior to start with passing score of 80% or higher; includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; updated annually

This position requires thorough compliance with all credentialing, health, and competency requirements prior to onboarding.
