Required Qualifications

• License: Current Surgical Technologist license or certification as required for the specialty.
• Certifications:

  • BLS (American Heart Association required)
• Experience:

Required Credentials and Compliance

• Health and Immunization Requirements:

  • Documented proof of two MMR vaccines or positive IGG titer (no declinations accepted).
  • Documentation of Hepatitis B vaccination series completion or signed declination.
  • Influenza vaccine for flu season (exemptions accepted with mask requirement).
  • Documentation of two Varicella vaccines or immunity evidence (no declinations accepted).
  • Tdap vaccine valid within 10 years (no declinations accepted).
  • TB screening: Positive IGRA or PPD results, negative chest X-ray within one year, and annual TB questionnaire.
  • Fit Test for respiratory protection, updated annually.
• Drug Screening: 12-panel drug screen within 30 days prior to start, including MRO review if applicable.
• Background and Verification Checks:

  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 30 days of assignment start.
  • EPLS (Excluded Parties List System) check within 90 days of start.
  • Employment verification for the past seven years.
  • Education verification relevant to clinical work within 90 days of start.
  • Professional references: at least two qualifying references prior to first assignment.
• Additional Documentation:

  • Driver’s License/State ID (must be current and valid).
  • Statement of good health or completed physical exam prior to first assignment, updated annually.
  • Ascension Confirmation of Assignment on agency letterhead with accurate start/end dates.
  • Forward-facing badge photo with white or blue background (no alterations or filters).
