Required Qualifications
- License: Current Surgical Technologist license or certification as required for the specialty.
- Certifications:
- BLS (American Heart Association required)
- Experience:

- Health and Immunization Requirements:
- Documented proof of two MMR vaccines or positive IGG titer (no declinations accepted).
- Documentation of Hepatitis B vaccination series completion or signed declination.
- Influenza vaccine for flu season (exemptions accepted with mask requirement).
- Documentation of two Varicella vaccines or immunity evidence (no declinations accepted).
- Tdap vaccine valid within 10 years (no declinations accepted).
- TB screening: Positive IGRA or PPD results, negative chest X-ray within one year, and annual TB questionnaire.
- Fit Test for respiratory protection, updated annually.
- Drug Screening: 12-panel drug screen within 30 days prior to start, including MRO review if applicable.
- Background and Verification Checks:
- Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 30 days of assignment start.
- EPLS (Excluded Parties List System) check within 90 days of start.
- Employment verification for the past seven years.
- Education verification relevant to clinical work within 90 days of start.
- Professional references: at least two qualifying references prior to first assignment.
- Additional Documentation:
- Driver’s License/State ID (must be current and valid).
- Statement of good health or completed physical exam prior to first assignment, updated annually.
- Ascension Confirmation of Assignment on agency letterhead with accurate start/end dates.
- Forward-facing badge photo with white or blue background (no alterations or filters).
