Job Details

- MMR (Measles, Mumps, Rubella) – proof of 2 vaccinations or IgG titer
- Hepatitis B vaccination series initiated prior to start (titers not accepted)
- Varicella vaccination or IgG titer proof
- 2 Step PPD or IGRA (Quantiferon/Tspot) with chest x-ray if positive (within last year for CT)
- Physical/Health Statement confirming fitness to work (exam within last year)
- Trinity 10 Panel Drug Screen completed within 30 days of start (NO THC)
Professional and Compliance Requirements
- Signed Trinity Code of Conduct Form
- Trinity I9 form completed by agency
- Confidentiality and System Security Agreement signed
- Integrity and Audit Services Form signed shortly before start
- Conflict of Interest Disclosure Form signed
- Criminal Background Check (Federal, Nationwide, County for last 7 years) dated within 30 days of start
- Education Verification (highest academic degree or diploma verified)
- EPLS (Excluded Parties List System) check dated within 30 days
- OIG (Office of Inspector General) exclusion check dated within 30 days
- Joint Commission CORE Exam within the last year
- NSO (National Sex Offender) check dated within 30 days
- System Access Form completed and uploaded one week before start
- OFAC (Office of Foreign Assets Control) sanctions check within 30 days
- Trinity Profile submission including resume, skills checklist (within a year), and 2 professional references
