Medical Social Worker

Overview

• Travel/Contract role in Castro Valley, California
• Day shift: 5x8, 8:00 AM–4:30 PM
• Every other weekend required
• Every other Sunday in the ED for 12 hours, 8:30 AM–8:30 PM
• 13-week contract

Required Qualifications

• Valid Master of Social Work license in California
• Master of Social Work (2 years of experience)
• Master of Social Work experience within the last year
• BLS - Basic Life Support issued by American Heart Association or American Red Cross
• Master's degree in social work
• Experience with discharge planning

Role Summary

• Provides biopsychosocial assessment, crisis intervention, short-term counseling, advocacy, and linkage to resources
• Supports transitions of care for patients and families/significant others across patient care settings
• Provides psycho-education, may facilitate support groups, and collaborates with the interdisciplinary team on discharge planning
• May provide education to staff and other hospital departments and participate on committees as requested

Responsibilities

• Completes psychosocial assessment and care planning
• Assesses discharge planning resources and suspected abuse or neglect
• Provides direct clinical services, including discharge planning/transitions of care, counseling, crisis intervention, education, support groups, and resource linkage
• Advocates for patient and family access to care, safety, rights, and culturally and linguistically competent care
• Assists with ethical issues and uses the chain of command or Bioethics Committee when appropriate
