Medical Surgical Registered Nurse - Travel/Contract

Location: Gallipolis, Ohio
Shift: Nights (3 shifts per week, 12 hours each, 7:00 PM – 7:00 AM)
Contract Length: 13 weeks

Job Details

• Unit: Medical Surgical Rehabilitation
• Patient Ratio: 1:6
• Charting System: Paragon (experience not required)
• Weekend Requirement: Every other weekend and holidays
• Floating: May be required depending on need
• Radius Rule: Must live within 50 miles
• Scrub Color: Any solid color scrubs
• Block Scheduling: Available on a case-by-case basis upon request

Required Qualifications

• Valid RN license in Ohio
• Med Surg experience: Minimum 1 year, with experience within the last year
• Certifications:

  • BLS (Basic Life Support) issued by the American Heart Association, required for all RNs
  • ACLS (Advanced Cardiac Life Support), required
• Education: Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Compliance & Documentation Requirements

• Government issued photo ID (must carry while on assignment)
• Social Security Card
• Background Check: 7-year criminal background check including SSN trace, counties lived/worked, statewide, national criminal, sex offender, FACIS Level 3, OFAC
• EPLS/GSA/SAM and HHS/OIG checks (updated monthly)
• Fingerprinting: BCI and FBI fingerprints required within 12 months of start, updated every 5 years; candidates may start with results pending but must have results on file within 30 days
• Education Verification (for allied candidates only)
• Physical Exam: Must clear candidate to work without restrictions; required within 6 months for acute care units
• Fit Mask: Required within 6 months of start (3M 1860 or 3M VFlex 1804, small or regular)
• Immunizations and Health Screenings:

  • Tdap within 10 years
  • Negative TB test or chest X-ray (Quantiferon Gold within 30 days acceptable; Tspot not accepted)
  • Annual TB questionnaire if history of positive TB test
  • Measles, Mumps, Rubella: Positive titer or 2 vaccines; booster or declination if negative/equivocal
  • Hepatitis B: Positive titer required or signed declination
  • Varicella: Positive titer or 2 vaccines; booster or declination if negative/equivocal
• Drug Testing: Negative 12-panel drug screen within 30 days of start
• Flu Vaccine: Required for flu season (9/1/2025 – 3/30/2026) or signed declination by 12/31/2025
• COVID-19: Proof of vaccination or signed declination required

Additional Requirements

• References: Two positive professional references related to similar clinical experience; one preferred from most recent employer
• Employment Application: Must include 3 years prior work history with at least 2 years clinical practice in acute care in a similar area
• Unit-specific skills checklist (updated annually)
• Facility-specific documents including confidentiality agreements, orientation acknowledgments, dress and appearance, code of conduct, and others must be completed prior to start
• Recent passport-style photo for badge (neutral background)
