Job Opportunity: Speech-Language Pathologist (Home Health)

Location: San Jose, California
Employment Type: Travel/Contract
Contract Length: 4 months (minimum for less experienced clinicians) to 6 months


Job Description

  • Provide home health therapy services primarily in various counties including Alameda, San Joaquin/Stanilaus, Santa Clara, Sonoma, and Marin.
  • Patient caseload is managed by weekly visits rather than daily patient numbers.
  • Minimum of 23-24 patient visits per week required to qualify for a 40-hour workweek guarantee.
  • Types of cases include Home Health, Wound Care, and Geriatric Home Health patients.
  • No weekend shifts required.
  • Typical shift: Day shift, 5 days a week, 8 hours per day (8:00 AM – 5:00 PM).
  • Mileage reimbursed from first patient to last patient visit (not to/from home).
  • Locals within a 50-mile radius are accepted.
  • Dress code: Any color scrubs and closed-toe shoes.
  • Charting must be completed onsite using Kinnser/Wellsky systems; no remote charting allowed.
  • Free residential parking available.

Required Qualifications

  • Valid Speech Language Pathologist license in California.
  • Minimum 2 years of experience as a Speech Language Pathologist.
  • Experience as a Speech Language Pathologist within the last year.
  • Experience in a Home Health setting.
  • Basic Life Support (BLS) certification.

Compliance and Documentation Requirements

Candidates must provide the following prior to starting:

  • Government-issued photo ID (Driver’s License or State ID).
  • Emergency Contact Form.
  • Background checks within 1 year, including SS Trace, County searches (7 years), Nationwide, and Sex Offender registry. Annual updates required for EPLS/GSA/SAM and HHS/OIG checks.
  • Certifications: BLS (required), ACLS and PALS preferred but not mandatory for this role.
  • State License primary source verification.
  • Physical exam within 6 months signed by MD/DO confirming freedom from active communicable diseases.
  • Immunizations or proof of immunity for:
    • Tetanus/Diphtheria/Tdap (within 10 years or exemption)
    • Measles, Mumps, Rubella (proof of titer or vaccination for those born in 1957 or later)
    • Hepatitis B (titer, vaccine series, or declination)
    • Varicella (titer, vaccination, history of chickenpox, or declination)
  • Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year; if past positive, negative chest X-ray within 5 years plus annual TB questionnaire.
  • Drug screening: 9-panel drug screen within 1 year of start (instant acceptable).
  • Seasonal flu vaccination or facility-specific declination form required during flu season (October 1 – March 31).
  • COVID-19 vaccination or approved medical/religious exemption form.
  • Auto insurance card (if applicable).
  • Two professional references required within 3 years for travel, allied, or per diem acute care clinicians.

Additional Notes

  • All required documents must be submitted before the clinician can begin patient care; compliance processing may take 2-3 weeks.
  • Requests for time off (RTO) must be submitted at the time of application; no additional RTO accepted after an offer is made.
  • Clinicians are expected to be punctual, patient-care focused, self-starters, and diligent in completing charting.
