Job Opportunity: Physical Therapist (PT) - Home Health
Location: Paducah, Kentucky (Territory includes Mayfield, Wickfield, Salem, Benton, Arlington, and nearby areas)
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Specialty: Home Health Physical Therapy
- Average Patients per Day: 5-7
- Types of Cases: Geriatric
- Guaranteed Hours: 40 hours/week
- Territory: Up to a 60-mile radius from the hub/office
- Locals accepted at travel rate
- Dress Code: Scrubs (any color), no jeans
- Equipment Provided: iPad
- Cell phone use: Not allowed
- Mileage reimbursed at IRS standard rate
Required Qualifications
- License: Valid Physical Therapist license in hand
- Experience: Minimum 1 year recent Physical Therapy experience (Home Health specialty)
- Certifications: BLS (Basic Life Support) required (AHA, ARC, Military accepted)
- Skills: OASIS experience required
- EMR: Experience with HCHB (Homecare Homebase) required
- Timely communication and documentation required
- No on-call responsibilities
Background and Compliance Requirements
- Comprehensive background checks required within 90 days of start, including:
- SSN Trace
- County searches for last 7 years of residence/work/school
- FACIS Level 1M
- Nationwide Sex Offender Registry
- Motor Vehicle Records (MVR)
- Education and Work History Verification
- State-specific searches for Kentucky (Elder Abuse, Nurse Aide Abuse, Child Abuse Registries)
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting may be required depending on state placement
- Education verification of highest degree related to profession
- NURSYS verification as applicable
- Competency exams as applicable (Medication Test/Pharmacology, Specialty Competency)
- Agency background attestation required
Health and Safety Requirements

- Annual TB questionnaire required
- Immunizations/vaccinations or documented declinations required for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- 9 Panel Drug Screen within last 12 months
- Seasonal flu vaccination or facility-specific declination form required during flu season
- COVID vaccination or facility-specific declination required
Additional Requirements
- Proof of current auto insurance required (due to driving responsibilities)
- Signed acknowledgment of Orientation Safety Manual (updated annually)
- Completion of facility-specific policies and forms prior to start
- 7 years of verifiable work history with no gaps required on employment application
- Two professional references within 2 years of start (travel/allied/per diem acute care)
