  • Ancillary Assistance: CNA/PCT available

  • Shift Hours: 8-hour shifts, schedule as requested
  • Weekend Requirement: Minimum every other weekend

Job Description

  • RN for LTC/SNF unit
  • Floating required but no call
  • Must work 8-hour shifts
  • Schedule adjusted to meet unit and patient care needs
  • No block schedules
  • Orientation: 2-3 days, bi-weekly on Mondays
  • Patient care modality: Long-term care / Skilled nursing facility
  • Basic LTC equipment; no respiratory or anesthesia support on unit

Required Qualifications

  • Valid Registered Nurse (RN) license in New York
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Minimum 1 year of Long Term Care (LTC) nursing experience
  • LTC nursing experience within the last year
  • Experience in Post Acute care settings
  • Experience in Rehabilitation settings

Orientation & Scheduling

  • Orientation: 2-3 days, bi-weekly on Mondays
  • Scheduling system: Smart Square with staffing resource center
  • No block schedules
  • Weekend requirement: Minimum every other weekend or adjusted schedule
  • Call: None
  • Shift length: 8 hours

Additional Requirements & Information

  • Reliable personal transportation required (no public transportation available)
  • Vehicles should have all-season or snow tires November through April
  • Dress code: Nursing scrubs (navy or white for aides), minimal jewelry, groomed hair/beard, nail polish per dress code
  • COVID-19 vaccination or declination required
  • Flu vaccination or declination required during flu season (Oct 1 - Mar 31)
  • Background checks, education verification, and various health documents required (see below)

Required Documentation & Compliance

  • Background Check within 30 days covering all locations lived/worked/schooled in past 7 years
  • Education Verification (based on job requirements)
  • Drug Testing within last 12 months (required for NY facilities)
  • Physical Exam within 12 months (required for VT facilities, recommended for NY)
  • Immunizations and Titers:
    • Tdap within 10 years or titer
    • Negative TB test or chest X-ray within 12 months
    • MMR (2 vaccines or positive titer)
    • Varicella (2 vaccines or positive titer)
    • Hepatitis B series and titer or signed declination
    • Pneumonia vaccination or declination (required for NY facilities)
  • N95 Fit Test prior to assignment and annually (acceptable mask types specified)
  • COVID-19 vaccination or declination
  • Flu vaccination or declination during flu season
  • References: 2 professional references from assignments within last 3 years
  • Skills Checklist within 12 months
  • Work History Verification: 2 years of most recent clinical work history
  • Various facility-specific forms and attestations (e.g., abuse registry consent, emergency contact, dress code policy review)

Summary

This is a travel contract RN position in a 34-bed skilled nursing unit within an acute care hospital in Plattsburgh, NY. The role requires at least 1 year of LTC/SNF nursing experience, a valid NY RN license (or proof of application), and standard certifications including BLS and ACLS. Floating to acute care as a sitter may be required, but no call responsibilities. Reliable transportation is essential due to lack of public transit. The contract length is approximately 16 weeks with 8-hour shifts and a minimum of every other weekend worked.
