Job Description

• Provide and coordinate home nursing care according to physician orders and agency policies.
• Supervise and evaluate Home Health Aides and other Licensed Practical/Vocational Nurses.
• Conduct home visits with an average patient load of 5-8 patients per day.
• Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
• Must have reliable transportation, a current driver’s license, and required liability insurance.
• Travel radius up to 75 miles from the home clinic.
• Use of electronic medical record system HomeCare HomeBase (HCHB) required; proficiency within the last 6 months preferred.
• Typical schedule: Monday to Friday, 8 AM to 5 PM; some positions may have alternate shift schedules (e.g., 4x10s Friday-Monday).
• Weekend and on-call requirements vary by location and should be discussed during the interview.
• Local candidates accepted.

Required Qualifications

• Valid LPN/LVN license in New Hampshire.
• Minimum of 1 year of recent experience in Home Health nursing, including experience within the last year.
• Experience working with adult patients in a Home Health setting.
• Hospice experience.
• BLS certification issued by the American Heart Association or American Red Cross.
• Proficiency with HomeCare HomeBase (HCHB) electronic medical record system.
• Reliable transportation and current driver’s license.
• Must have agency-required liability insurance.
• Ability and willingness to travel up to 75 miles from home clinic.

Additional Information

• No guaranteed hours; patient census may fluctuate.
• Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).
• Dress code: Scrubs.
• Productivity expectations: Approximately 32 units per week (about 7 patients per day).
• Background checks, drug screening, and various state-specific compliance requirements apply.
• Physical exam, immunizations, and health screenings (including TB, MMR, Hepatitis B, Varicella, Flu, COVID) must be current and compliant.
• References and work history verification required.
• Candidates must complete required compliance and confidentiality training prior to start.
