Physical Therapist - Home Health
Location: Lacey, Washington
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Specialty: Physical Therapy (PT) in Home Health
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM (Day shift, 5x8 hours)
- Guaranteed 40 hours per week
- Average caseload: 5-7 patients per day (geriatric population)
- On-call required every 6th weekend
- Travel radius: up to 60 miles from the office hub
- Dress code: Scrubs (any color), no jeans
- Documentation and communication must be timely
- iPad provided for work; cell phone use not allowed
- Mileage reimbursed at IRS standard rate
- Flexibility with territory expected
- First-time travel therapists not accepted
- Local candidates not accepted
Required Qualifications
- Valid Physical Therapist license in Washington
- Minimum 1 year of Physical Therapist experience within the last year
- Basic Life Support (BLS) certification (AHA, ARC, or Military accepted)
- Experience with OASIS documentation
Compliance and Background Requirements
- Background checks within 90 days of start, including:
- SSN Trace
- County searches for last 7 years
- FACIS Level 1
- Nationwide Sex Offender Registry
- Motor Vehicle Record (MVR)
- Education verification (highest degree related to profession)
- Work history verification (7 years)
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting required if candidate has not lived in Washington for the past 3 years (completed on-site)
- State-specific criminal history and employability status checks as applicable
- Signed Orientation Safety Manual Acknowledgement (updated annually)
- Agency Background Attestation required prior to start
Health and Immunization Requirements
- Physical exam within the last 12 months (physician statement)
- Fit mask test (N95 accepted)
- Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD
- Annual TB questionnaire required
- Immunizations or positive titers for:
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- Seasonal flu vaccination or facility-specific declination form required for flu season
- COVID vaccination or facility-specific declination required
- 9-panel drug screen within 12 months of start (instant acceptable)
Additional Requirements
- Proof of current auto insurance required (due to driving responsibilities)
- Must provide 2 professional references from travel, allied, or per diem acute care within 2 years of start
- Full 7 years of work history documented with no gaps in employment application
- Annual skills checklist and competency exams as applicable
Notes
- Prior travel experience is required (first-time travelers not accepted)
- Candidate must be flexible with travel territory and on-call schedule
- No cell phone use allowed during work hours; iPad provided for documentation and communication
