Job Details
- Start Date: December 1, 2025
- Shift: 8-hour day shifts (No 4x10 or 3x12 shifts available)
- Patient Ratio: 1:1 (May visit up to 6 patients per day; 30 productivity points standard)
- Experience Required: Minimum 1 year of skilled home health experience
- First-Time Travelers: Accepted
- Weekend Work: Not required
- Certifications Required: BCLS (Basic Cardiac Life Support)
- On-Call: Not required
- Floating: Not required
- Block Scheduling: Not accommodated
- Locals Accepted: Yes, within 50-75 mile radius
- Pending License: Accepted
- Transportation: Traveler must have their own transportation
- Travel Radius: Up to approximately 60 miles per day (60-90 minutes total drive time)
- Documentation System: EPIC Remote Client (laptop), Rover (phone app), and OASIS documentation required
- Guaranteed Hours: Facility may call off up to one shift every two weeks
Required Experience
- Specialty Experience: At least 1 year of skilled home health experience (highest requirement found)
- Prior Travel Experience: Not required
Certifications & Licenses
- Basic Life Support (BLS): Required
- Other Certifications: ACLS, PALS, NRP, Chemotherapy, EFM, IV Certification, MAB, NIH Stroke, CPI may be required depending on UD (User Data) or clinical track but not specifically required for this role
- State Licensure: Must be licensed to work in Oregon; if working in border units, licensure for both Oregon and Washington may be required
- Pending License: Accepted
Compliance & Background Requirements
- Background check within 45 days of start, covering all locations lived/worked in past 7 years
- Various state-specific background checks and registries as applicable (e.g., Oregon Abuse/Neglect, Washington WATCH, OBRA Registry for CNAs)
- Drug screening: 10-panel drug screen within 90 days of start (no instant tests)
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO
- Immunizations and health documents required:
- TDAP (within 10 years or declination)
- Negative TB test or chest X-ray within 12 months
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine or declination (required during flu season)
- COVID vaccine or declination
- Fit mask test required for travel candidates (updated annually)
- Respiratory safety clearance required annually for travel candidates
Training & Competency Requirements
- Completion of Providence Health and Services WorkRamp modules prior to start, including:
- Core regulatory modules (annual)
- Specialty-specific modules for home health and therapy/rehabilitation roles
- Epic training modules (completion required within a week of orientation)
- Competency exams and medication/pharmacology tests as applicable
- Skills checklists updated annually as applicable
Additional Notes
- Attire: Business casual preferred; scrubs allowed but no specific color required; no jeans
- Parking: Free
- Mileage reimbursement: IRS standard
- Traveler expected to minimize windshield time; system groups clients to reduce travel time
- References: Minimum 2 supervisor references covering 1 year of employment in a comparable specialty/setting within the last 3 years
- Employment application required with at least 5 years of employment history
