Location: Tulsa, Oklahoma
Employment Type: Travel/Contract

Position Details

  • Specialty: Therapy/Rehabilitation (Home Health)
  • Minimum Years of Experience Required: 1 year (recent experience required)
  • Prior Travel Experience: Not explicitly required

  • Comprehensive background checks required within 90 days of start, including:
    • SSN Trace
    • County searches for last 7 years of residence/work/school
    • FACIS Level 1M
    • Nationwide Sex Offender Registry
    • Motor Vehicle Records (MVR)
    • Education and Work History Verification
    • State-specific searches as applicable (e.g., Kentucky, Virginia, Indiana)
  • EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
  • Fingerprinting required depending on state placement (e.g., Ohio, Connecticut) and must be current within 12 months
  • Education verification of highest degree related to profession required

Certifications & Competencies

  • BLS (AHA, ARC, Military accepted)
  • Orientation Safety Manual Acknowledgement (updated annually)
  • State License verification (primary source within 90 days of start and upon expiration)
  • NURSYS verification for applicable professions
  • Medication Test/Pharmacology Competency Exam as applicable
  • Competency Exam as applicable to specialty

Facility-Specific & Health Requirements

  • Proof of current auto insurance required (for positions involving driving)
  • Completion of Acceptable Use Policy prior to start
  • Flu and COVID vaccination or facility-specific declination forms required
  • Physical exam within last 12 months (not required annually)
  • Negative TB test or chest X-ray within required timeframes
  • Annual TB Questionnaire required
  • Immunizations or titers for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • 9 Panel Drug Screen within last 12 months (instant acceptable)

Employment History & References

  • Full 7 years of work history required on application with no gaps
  • Employment history verification for 7 years required within 90 days of start
  • Professional references required:
    • 2 for Travel, Allied, Per Diem Acute Care
    • 1 for Per Diem Non-Acute Care
  • References must be recent (within 2 years) and for some states completed within 90 days of start
