Experience & Certification Requirements

  • Years of Experience Required: Minimum 1 year in specialty
  • First-timers Accepted: Yes

  • Certifications Required:
    • CSFA (Certified Surgical First Assistant) required
    • NBSTSA preferred
    • BLS (Basic Life Support) required
  • On-call: Yes, one shift per week
  • Weekend Requirement: To be discussed during interview

Compliance & Documentation Requirements

  • Licenses & Verifications:
    • Active Driver’s License required
    • Primary source verification of state license required
    • NURSYS verification for RN/LVN required
    • Florida AHCA Level II fingerprint clearance required prior to start
  • Background Checks:
    • Standard background check within 1 year of start, including SS trace, county searches, nationwide, sex offender, education verification
    • EPLS/GSA/SAM and HHS/OIG checks required
    • Foreign equivalency evaluation required if education is outside USA or Puerto Rico
  • Education Verification: Required for all healthcare professionals
  • Health Documents:
    • Physical exam within 6 months of start
    • Negative TB test or chest X-ray within 10 weeks of start (annual TB questionnaire required)
    • Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella
    • Drug testing: Standard 9-panel within 30 days (instant tests not accepted)
    • Seasonal flu vaccination or declination accepted (mask required if declined)
    • COVID-19 vaccination required (exemptions accepted)
    • Respirator medical clearance if applicable
  • Competency Exams:
    • Medication/Pharmacology competency exam (score 80%+) as applicable
    • Color blind testing (Relias) required prior to start
    • Dysrhythmia/EKG competency exam required for RNs in certain units
  • References: Two references from past 2 years required

  • Photo for Badge: Professional photo required prior to start
  • Facility Specific:
    • Dress code and tobacco policy acknowledgment required
    • Completion of BayCare Online Learning Center modules prior to start (4-8 hours)
