Location: Farmington, Missouri (coverage area includes Fredericktown to Bonne Terre and occasionally further north)

Requirements

• Experience: Minimum 1 year of experience as a Certified Occupational Therapy Assistant
• Prior Travel Experience: Not required (first-timers accepted)
• Certifications:

  • Basic Life Support (BLS) required
• Licensing:

  • Pending license accepted
• Schedule:

  • No weekend requirement
  • No on-call required
  • No block schedule accommodation
• Local Candidates: Accepted within local radius
• Return to Office (RTO) Restrictions: No more than 7 days of RTO per 13-week contract
• Guaranteed Hours: Minimum 3 shifts per 13 weeks (cancelled shifts must be full shift length)

Compliance & Documentation Requirements

Background Checks

• Criminal background check within 6 months (7-year search including statewide, nationwide, and international if applicable)
• EPLS/GSA/SAM and HHS/OIG checks quarterly
• Education verification of highest related education required
• State-specific searches and registries as applicable (Missouri and Illinois)

Certifications & Verifications

• BLS (Basic Life Support) certification required
• Other certifications may be required depending on unit (e.g., ACLS, PALS, NRP, CPI/MAB) – check unit-specific requirements
• State license primary source verification required (Missouri licenses verified via NURSYS)
• IV Certification required for LPNs/LVNs in Missouri facilities (not applicable here but noted for reference)

Health & Immunization

• Physical exam within 1 year of start
• Fit mask test required and must be renewed annually
• Tetanus/Diphtheria/Tdap vaccine or declination required
• Negative TB test or chest X-ray within 1 year (two negative PPDs or equivalent)
• Immunizations or proof of immunity for Rubeola (Measles), Mumps, Rubella, Varicella
• Drug testing within 1 year of start, must include specific panels and be valid throughout assignment
• Seasonal flu vaccination or approved exemption required during flu season (09/30 - 04/01)
