Job Opportunity: PT Home Health (Therapy/Rehabilitation)

Location: San Jose, California
Employment Type: Travel/Contract

Position Details

Specialty: Physical Therapist (PT) in Home Health
Patient Focus: Home Health, Wound Care, Geriatric Home Health Patients
Average Visits: Minimum 23-24 visits per week to qualify for a 40-hour guarantee
Territory: Multiple counties in California (specific territory discussed during interview; may include surrounding counties)
Locals Accepted: Yes, within a 50-mile radius

Experience Requirements

Years of Experience:
- Minimum 1 year in Home Health
- Minimum 2 years overall as a clinician (higher value applies)
Prior Travel Experience: Not required
First-time Travelers: Accepted

Contract Details

Length: 4-6 months per contract
- Less experienced clinicians must commit to 6 months (non-negotiable)
Weekend Requirement: No weekends required
On-Call: Not required (discretionary)

Certifications & Licenses

Required at Submission:
- Valid State License
- BLS (Basic Life Support) certification
Preferred/Additional Certifications:
- ACLS (Advanced Cardiac Life Support)
- PALS (Pediatric Advanced Life Support)
Pending License: Not accepted

Compliance & Documentation Requirements

Must have all required documents before starting (compliance process may take 2-3 weeks)
Required documents include but are not limited to:
- Government-issued photo ID (Driver’s License/State ID)
- Background checks (within 1 year, including SS Trace, County searches, Nationwide Sex Offender)
- Annual EPLS/GSA/SAM and HHS/OIG checks
- Physical exam within 6 months, including statement of no active communicable diseases
- Immunizations and titers:
  - Tetanus/Diphtheria/Tdap (within 10 years or exemption)
  - Negative TB test or chest X-ray (within 1 year)
  - Measles, Mumps, Rubella (proof of immunity or vaccination)
  - Hepatitis B (titer, declination, or vaccine series)
  - Varicella (titer, vaccination, or history)
- Drug screening (9-panel within 1 year)
- Seasonal Flu vaccination or declination (required during flu season)
- COVID vaccination or approved medical/religious exemption
- Emergency contact form
- Auto insurance card (if applicable)
Charting must be completed onsite using Kinnser/Wellsky systems; no remote charting allowed

Work Environment & Expectations

Dress code: Any color scrubs, closed-toe shoes
Free residential parking available
Mileage reimbursed for travel between patients (not to/from home)
Covid vaccination required; declination allowed for religious reasons
Flu vaccination declination allowed with facility-specific form
