Case Manager

Overview

• Travel/Contract role in Tucson, Arizona
• 13-week assignment
• Day shift, Monday–Friday, 8:00–16:30
• Weekend coverage is required once every 4 weeks, with a weekday off for compensation
• No large RTO gaps within the 13-week assignment

Required Qualifications

• Valid RN license in Arizona
• Associate of Science in Nursing / Associate Degree in Nursing
• 2 years of case management experience
• Case management experience within the last year
• 2 years or more of total travel experience
• Acute care experience
• Experience working with adult patients
• Experience in an inpatient setting
• Experience in Adult Emergency Department
• Discharge planning experience
• Epic experience
• 2 professional references from employment within the last 3 years, including 1 supervisory reference

Preferred Qualifications

• Knowledge of Medicare/Medicaid/insurance coverage
• Familiarity with CMS and CoP

Responsibilities

• Manage all aspects of transitional care planning from admission to discharge
• Complete initial case management assessment interviews
• Provide transitional care follow-up as needed
• Ensure safe discharge planning to the patient’s final destination
• Collaborate with community partners and vendors
• Multitask, prioritize, document accurately, and work with behavioral health facilities and specialists as needed

Additional Notes

• Some departments require ACLS and PALS
• Mental health support/therapy is handled by specialized teams, not this role
