Overview
- Travel/contract home health role in Bloomington, Indiana
- 13-week assignment
- Day shift, 5x8 hours, 8:00–17:00
- Home visits with periodic office meetings
- Territory includes travel up to 75 miles from the home clinic
- Local candidates are accepted
Required Qualifications
- Minimum 1 full year of Adult Home Health experience or 1 full year of Hospice experience, depending on the specialty
- Recent specialty experience
- Current CPR/BLS
- Must have a valid license in hand
- Reliable transportation
- Current driver’s license
- Agency-required liability insurance
- Proficiency with HomeCare HomeBase within the last 6 months
- Home health experience
Preferred
- Potential to hire if available and a good team fit
Assignment Details
- Average patient load: 5–8 patients per day
- Productivity expectation: 30 points per week
- Weekly travel: about 300 miles, depending on where the contractor lives during the assignment
- On-call: 1 week out of 4, Monday–Thursday night call
- Scrubs required

- First-time travelers are not accepted
- Guaranteed hours are not provided
- OASIS proficiency is required for RNs who have used OASIS within the last 6 months; if not used within the last 6 months, retraining is required
