Job Overview

• Location: Lake Zurich, Illinois
• Employment type: Travel/Contract

• Schedule: Day shift, 5x8 hours, 8:00 AM–5:00 PM
• Contract length: 13 weeks
• Hours: 40 per week
• Patient volume: About 5–6 patients per day
• Territory: Up to 75 miles from the home clinic

Responsibilities

• Home visits
• Periodic office meetings
• Start of care and follow-up visits
• Travel between patient homes

Required Qualifications

• Valid Physical Therapist license in Illinois
• Physical Therapist (1 year of experience)
• Physical Therapist experience within the last year
• One completed travel assignment
• BLS issued by American Heart Association or American Red Cross
• Experience in a Home Health setting
• Driver's License
