Job Opportunity: Home Health Physical Therapist

Location: West Branch, Michigan
Employment Type: Clinical Allied Health


Job Summary

Provide medically prescribed, goal-oriented physical therapy in accordance with the Physician Plan of Care, company policies, and professional standards. Responsibilities include patient assessments, development and implementation of physical therapy plans of care, documentation, patient and family education, and participation in interdisciplinary team meetings. Supervise Physical Therapist Assistants and aides as needed.


Key Responsibilities

  • Develop and implement patient physical therapy plans of care.
  • Conduct patient visits for assessments and evaluations; communicate with physicians as needed.
  • Document care accurately and timely to maintain patient medical records.
  • Provide education and instruction to patients, families, and team members.
  • Participate in quality assessment and performance improvement programs.
  • Apply agency policies and comply with federal and state regulations.
  • Provide skilled restorative treatments including pain reduction, gait training, ultrasound, electrical muscle stimulation, prosthetic training, balance and coordination training, cardiovascular rehab, and home safety instruction.
  • Perform initial and ongoing comprehensive patient assessments including OASIS assessments.
  • Evaluate outcomes of care.
  • Supervise Physical Therapist Assistants, Certified Nursing Aides, and Home Health Aides; provide supervisory visits as necessary.

Contract Length

Approximately 13 weeks


Required Qualifications

  • Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
  • Current Physical Therapist license in Michigan, in good standing with the state licensing board.
  • Valid driver’s license and current automobile insurance.
  • Knowledge of physical therapy principles and methodology appropriate for terminally ill patients.
  • Strong problem-solving and decision-making skills.
  • Ability to manage patient care including symptom management, crisis intervention, and family intervention.
  • Excellent communication skills and ability to develop rapport with patients, families, and staff.

Preferred Qualifications

  • One year or more of physical therapy experience in a multidisciplinary setting.
