Job Summary

Evaluate and treat referred occupational therapy patients according to physician’s orders.

Job Responsibilities

• Perform initial patient evaluations, including administering appropriate tests and measures.
• Identify impairments and functional limitations.
• Establish initial treatment plans in collaboration with patients and caregivers.
• Select and administer treatment strategies based on physician orders and treatment plans.
• Monitor and document functional improvements and goal attainment regularly.
• Update treatment plans and goals according to department standards.
• Complete documentation timely and accurately, reflecting skilled therapeutic interventions.
• Maintain productivity standards and perform duties efficiently.
• Utilize non-productive time to support department needs.
• For Home Health Care, comply with applicable rules, regulations, and National Patient Safety Standards.
• Use resources responsibly and participate in department performance improvement activities.

Required Qualifications

• Valid Occupational Therapist license in Wisconsin
• Minimum 1 year of occupational therapy experience within the last year
• American Heart Association (AHA) Basic Life Support (BLS) for Healthcare Provider or equivalent (must obtain within 30 days of hire)
