Admission and Transfer RN

Role Overview

• Travel Registered Nurse role in the Admission and Transfer Center.
• Primary location: Corporate Office.
• Start date: ASAP.
• Contract length: 13 weeks.
• Guaranteed hours: 36 hours per week.
• Shift: 12-hour days or nights with variable start times (7am, 11am, 1pm, and 7pm).
• On-call/call-back: May be low censused and put on call; call-back minimum is 2 hours.
• Weekend/holiday: Every other weekend and every other holiday.
• Float: NA.

Required Qualifications

• Wisconsin RN license.
• AHA or ARC BLS.
• 3–5 years of ICU or ER experience.
• Trauma level II experience.
• EPIC experience.
• Must comply with flu/COVID masking policy if declining vaccination during flu season.

Preferred Qualifications

• 5 years of experience preferred.
• Transfer Call Center experience highly preferred.

Key Responsibilities

• Facilitates consults and transfers for patient placement.
• Coordinates patient admissions from other healthcare facilities and physician offices/clinics.
• Coordinates patient transfers across disciplines, including disposition, ambulance transportation, and specialist referrals.
• Provides estimated time of arrival to referring and accepting sites.
• Documents and tracks patient transfers into and out of the system.
• Supports movement of patients for leave-of-absence procedures or testing.
• Collaborates with bed planning teams and monitors bed availability.
• Uses critical thinking and triage skills to recommend appropriate facility transfer based on acuity, service needs, condition, and facility capacity.
• Communicates patient condition to sending or receiving providers and facilities.
• Advocates for priority admission or transfer to a higher level of care.
