Job Opportunity: Registered Nurse - Home Health (Travel)

Job Description

• Provide home health nursing care primarily in the Stevens Point and Plover areas.
• Manage a patient load of approximately 20-25 patients, averaging 4.5 visits per day.
• Travel distances vary from 25 to 60 miles per day depending on territory and patient census.
• Variable shifts Monday through Friday, 5-8 hours per day.
• Float to other territories as needed during low patient census.
• On-call responsibilities 1-2 times per week and every other weekend, including holiday rotation.
• On-call duties primarily involve triaging calls with a 15-minute response time.
• Documentation and charting using OASIS (required) and Epic (preferred).
• Clinical duties include wound care, IVs, ports, lab draws, chronic disease management, and coordination of care.
• Must be adaptable, dependable, compassionate, and able to manage change well.

Requirements

• Minimum 2+ years of home health nursing experience (preferred).
• Prior travel experience required.
• Active Wisconsin or Compact nursing license.
• BLS certification.
• Valid driver’s license.
• OASIS certification is a plus.
• Must comply with flu and COVID vaccination policies or wear a mask during flu season.
• Ability to work independently and manage patient care in a home setting.

Work Hours & Other Details

• Guaranteed 38 hours per week with flexibility up to 40 hours.
• May be assigned up to 36 hours of unbillable low census hours.
• Call-back pay available with a minimum of 2 hours.
• No guaranteed orientation week.
• Scrub color is flexible.
