Physical Therapist - Georgia

Location: Dallas, Georgia
Employment Type: Long Term
Contract Length: 13 weeks
Schedule: Monday - Friday
Guaranteed Work Week: 40 hours

Job Description

Provide physical therapy services primarily within Fayette and Fulton counties, with possible coverage up to a 60-mile radius from the facility.
Average caseload: 4-6 visits per day (PT/OT/SLP).
Productivity expectations: 28-32 points per week.
Visit types and point values include:
- Start of Care (SOC): 2.5 points
- Resumption of Care (ROC): 2 points
- Evaluations/Recertifications/Discharges (OASIS): 1.5 points
- Discipline Discharge, routine visits, 30-day reassessments: 1 point
- Weekly case conferences: 1 point
Overtime requires pre-approval.
Two weeks cancellation notice required.

Required Qualifications

Valid Physical Therapist license in Georgia
Experience in a Home Health setting
Experience with Homecare Homebase (HCHB) documentation
Experience with OASIS

Notes

May be asked to assist in other territories within the service area as needed.
