Work Schedule

• Monday to Friday
• 8:00 AM to 4:30 PM
• 40 hours per week (32 hours guaranteed)
• On-call and weekend availability required

Job Responsibilities

• Evaluate patients’ functional status and physical therapy needs according to CMS documentation requirements.
• Consult with physicians to develop therapy plans of care.
• Provide physical therapy services within the scope of state laws and plan of care.
• Observe and report patient responses and condition changes to nurse supervisors and/or physicians.
• Treat patients to relieve pain, restore function, and maintain maximum functioning using evidence-based techniques.
• Educate healthcare team members, patients, and caregivers about the plan of care, functional limitations, and progress.
• Visit patients throughout the agency’s service area.
• Complete progress notes for each visit and submit according to policy.
• Complete patient evaluations and develop plans of care within state guidelines.
• Report evaluation outcomes, goals, and projected frequency of care to nurse supervisor within 24 hours.

Required Qualifications

• Valid Physical Therapist license in Massachusetts
• BLS certification
• Bachelor’s Degree
• Valid driver’s license

Education

Bachelor’s Degree (minimum)

Preferred Skills and Attributes

• Ability to work independently
• Quick learner with fast onboarding capability
• Strong patient skills in home care, especially post-acute hospitalization and assisted living populations

Dress Code

Business casual (no jeans), no specific scrub color required
