Job Opportunity: LTC/LPN - Sheridan, Wyoming
Position Details
- Specialty: LTC/LPN
- Location: Sheridan, Wyoming
- Shift: Nights, 12-hour shifts
- Contract Length: 13 weeks
Qualifications & Requirements
- Current unrestricted Wyoming Practical or Vocational Nurse Licensure.
- Minimum 2 years LPN experience required.
- BLS (AHA) certification required.
- LPN IV certification (IV-C) recognized by Wyoming State Board of Nursing required within 1 year of position start date.
- Additional certifications may be required based on unit assignment.
Job Responsibilities
- Provide direct and indirect patient care under the supervision of a Registered Nurse.
- Communicate effectively with physicians, managers, and co-workers regarding patient condition and diagnostic results.
- Prioritize care based on physical and psychosocial patient needs.
- Respond quickly and accurately to changes in patient condition or treatment response.
- Perform general nursing duties with adequate supervision.
Essential Functions
Professional Practice
- Adhere to organizational policies and professional nursing standards (Wyoming State Board of Nursing, ANA Scope and Standards).
- Collaborate professionally with other departments to provide patient-centered care.
- Demonstrate communication, conflict resolution, team building, and leadership skills.
- Comply with organizational service standards.
Quality of Care
- Perform patient care in a safe environment minimizing errors.
- Conduct head-to-toe assessments and reassessments as per policy.
- Develop and revise individualized care plans based on patient response.
- Assess and manage pain using pharmacological and non-pharmacological methods.
- Coordinate and supervise patient care.
- Develop and evaluate patient and family teaching plans.
- Maintain documentation according to standards.
- Perform treatments and services within scope of practice.
- Communicate clearly with healthcare team members.
- Consult other departments for interdisciplinary care.
- Provide age- and disease-appropriate care.
- Assist physicians with procedures requiring technical/manual skills.
- Report significant events affecting patient safety promptly.
Service
- Treat patients and families with respect and dignity.
- Address psychosocial, cultural, ethnic, and spiritual needs.
- Act as liaison between patients/families, physicians, healthcare providers, and administration.
- Involve patients and families in care planning.
- Maintain good working relationships within and across departments.
- Identify and address psychosocial needs; coordinate with social services/discharge planners.
- Demonstrate flexibility, organization, and ability to function well under stress.
