Job Description

Seeking an experienced Home Health Physical Therapist traveler to provide physical therapy services according to a physician's plan of care. Responsibilities include:

• Assessing, evaluating, and updating therapeutic/rehabilitative/functional status
• Evaluating home environment and patient mobility, muscle strength, gait, and range of motion
• Directing physical therapy treatment and instructing patients, families, and caregivers on therapeutic appliance use
• Reporting patient reactions and changes in condition to the physician
• Initiating physical therapy programs and instructing other personnel or family members involved in patient care
• Supervising homemaker/Home Health Aides participating in rehabilitation
• Identifying patient and family needs for other home health services and making referrals
• Preparing and submitting clinical and progress summaries
• Providing physical therapy consultation and inservice education programs as needed
• Participating in peer consultation and ongoing staff competency assessments
• Complying with Title 22 Medicare Conditions of Participation, Medi-Cal, OSHA, and other relevant regulations

Required Qualifications

• Minimum 2 years of experience in healthcare; Home Health experience including OASIS strongly preferred
• Active California Physical Therapist (PT) license
• Current BLS certification
• Active NPI number
• Valid driver’s license and proof of automobile insurance
• Good oral and written communication skills and strong organizational skills
• EMR HomeCare Home Base (HCHB) experience preferred

Additional Information

• Candidates may not have been employed (full time, part time, or PRN) by any facility associated with CommonSpirit, including CHI and/or Dignity, within the past year (PRN employment through outside firms is acceptable)
• Two supervisory references required at submittal (one within 12 months, one within 3 years)
• Work schedule: 8-hour days, every other week (EOW)
