Hospice/Outpatient Bereavement Coordinator
Location: Albany, Oregon
Employment Type: Contract
Contract Length: 13 weeks
Shift: Days (8:00 AM – 4:30 PM), occasional weekends
Hours per Week: 24 hours (0.6 FTE)
Job Description
Provide support to patients and family members through the final stages of illness in a hospice setting. Services are delivered in patients' homes, skilled nursing facilities, assisted living facilities, and adult foster homes. Responsibilities include grief and loss counseling, pain and symptom management, medical social services, counseling, spiritual support, hospice aide coordination, volunteer support, and bereavement services.
Required Qualifications
- Valid Bereavement Specialist license in Oregon
- Minimum of 1 year of Bereavement Specialist experience, including experience within the last year
- Bachelor’s degree in social services, counseling, psychology, or a related field
- Social Worker experience
- Healthcare Provider Level BLS certification
- Valid driver’s license, personal automobile, and auto liability/property damage insurance required; must maintain vehicle in good working order

- Position requires occasional weekend work
- Must be able to travel to various patient locations within multiple counties
