Rehabilitation Therapy Contract Job in Hilo, Hawaii

Overview

• Location: Hilo, Hawaii
• Setting: Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)
• Employment type: Contract
• Contract length: 13 weeks
• Schedule: Day shift; generally 5 days per week, with likely one weekend day and variable 7-day departmental coverage
• Work expectations: May float to other units or facilities based on census and staffing needs
• Holiday coverage: Must be available to work Hawaii State and Federal Holidays as needed

Required Qualifications

• Valid Certified Occupational Therapy Assistant license in Hawaii
• Certified Occupational Therapy Assistant (2 years of experience)
• Certified Occupational Therapy Assistant experience within the last year
• BLS

Responsibilities

• Provide rehabilitation services in an SNF and/or ICF
• Perform evaluations, reassessments, treatments, and other clinical services within licensure and scope of practice
• Develop, implement, or carry out individualized treatment plans with the supervising therapist, physician, and interdisciplinary team as appropriate
• Deliver evidence-based therapeutic interventions to support function, independence, communication, mobility, swallowing, and quality of life as applicable
• Educate residents, families, and caregivers on treatment plans and patient progress
• Complete accurate, timely, and compliant documentation, including progress notes, plan-of-care changes, and discharges
• Supervise therapy assistants when applicable to the role

Training and Onboarding

• Orientation is provided for the rehabilitation department, documentation workflow, and facility processes
• Must arrive the Thursday before the start date to complete an employee health appointment on Friday
