Home Care Physical Therapist

Location: Barre, Vermont
Employment Type: Contract (16 weeks)
Shift: Monday to Friday with Weekend Rotation (8am-4pm)
Hours per Week: 36

Job Summary

Provide skilled physical therapy services in a home care setting, focusing on individualized care plans to maximize client mobility and independence. Collaborate with an interdisciplinary team to meet client needs and utilize various therapeutic modalities and standard therapy measurement techniques (ROM, strength, balance tests).

Required Qualifications

• Valid Physical Therapist license in Vermont
• Minimum 1 year of Physical Therapist experience, including experience within the last year
• Experience in a Home Health setting
• BLS certification

Experience

• Minimum 1 year of experience in Rehabilitation Therapy (Physical Therapy)
• Prior travel experience is not specified or required

• Work schedule includes weekend rotation
